  • Understanding Posterior Tibial Tendon Dysfunction (PTTD)

The Posterior Tibial Tendon is one of the most important structures in your foot. It runs down the inside of your ankle and is essentially the main engine that holds your arch up while you walk. When this tendon becomes inflamed, torn, or simply overworked, it can lead to a condition called Posterior Tibial Tendon Dysfunction (PTTD). Often starting with a subtle ache along the inside of the foot and ankle, PTTD is easily dismissed as just a sprain or general foot fatigue. However, if left untreated, this condition can weaken the tendon so much that it can no longer support your arch, leading to what we call Adult-Acquired Flatfoot.

The hallmark sign that your simple ache might be something more is the gradual flattening of your foot. As the arch collapses, the pain often worsens, making walking, running, or even standing for long periods incredibly difficult. This is why early intervention is absolutely crucial. Waiting until the arch has fully dropped means treatment becomes more complex, requiring longer rehabilitation times. At Thorpes Physiotherapy, we focus on catching this issue in its early stages through comprehensive examinations and professional gait analysis.

The good news is that PTTD is highly treatable, especially when managed proactively. Treatment focuses on reducing inflammation and providing the necessary support to take the strain off the failing tendon. This often includes targeted stretching and strengthening exercises, anti-inflammatories, and most importantly, customised support.
