When Your Head Hurts: Understanding Headaches and How Physiotherapy Can Help

Headaches are one of those health complaints that almost everyone experiences at some point, yet they can range from a mild, forgettable nuisance to a debilitating condition that shapes daily life.   • Cervicogenic headaches — headaches that originate from the structures in the upper neck

Occasionally, headaches can also be a sign of something more serious that requires urgent medical attention. Sudden, severe headaches unlike anything you’ve experienced before, headaches accompanied by fever, confusion, vision loss, weakness, or headaches following a head injury should always be assessed by a medical professional promptly.

For many people, however, headaches are recurring, predictable, and closely linked to the neck and upper back, and this is where physiotherapy can offer real, lasting help.

What Is a Cervicogenic Headache?

Cervicogenic headaches are headaches that are referred from the upper part of the neck. The joints, muscles, and nerves at the top of the cervical spine are closely connected to the nerves that supply sensation to the head — meaning that irritation or dysfunction in the neck can genuinely be felt as pain in the head, often around the base of the skull, temples, or behind the eyes.

These headaches are frequently mistaken for tension headaches or migraines, which is part of why they can be so tricky to diagnose and treat effectively. The good news is that because the source is musculoskeletal, physiotherapy is often very well suited to help.

Why Specialist Training Matters

Treating cervicogenic headaches isn’t always straightforward. Distinguishing between different headache types, correctly identifying the structures involved, and applying the right treatment approach requires a thorough, skilled assessment, this isn’t an area where a generic approach tends to work well.

How Physiotherapy Can Help with Headaches

If your headaches are found to be linked to the neck, physiotherapy can help in several ways, often combined as part of a personalised treatment plan:

Hands-on treatment for stiff joints. The small joints at the top of the neck can become stiff or restricted, particularly with prolonged sitting, poor posture, or previous injury. Gentle, skilled manual therapy can help restore normal movement to these joints, which may reduce the referral of pain into the head.

Releasing tight or overactive muscles. Muscles at the base of the skull and through the neck and shoulders can become tight and overactive, contributing to headache symptoms. Targeted hands-on techniques can help ease this tension and improve comfort.

Addressing irritated nerves. Sometimes, nerves in the neck become sensitised or irritated, amplifying pain signals. Postural and biomechanical analysis. How you sit, stand, and move throughout the day can place ongoing strain on the neck. A thorough biomechanical and postural assessment helps identify contributing habits or movement patterns, whether that’s related to desk setup, driving posture, or how you carry your bags, so we can address the cause, not just the symptom.

Guided exercise and movement. Once we understand what’s driving your headaches, we can guide you through specific exercises to improve strength, control, and mobility in the neck and upper back, helping to reduce the frequency and intensity of symptoms over time.

Don’t Forget About Stress.  It’s also important to acknowledge that stress can play a significant role in headache frequency and intensity, whether through increased muscle tension, poor sleep, or simply the body’s heightened response to daily pressures.
